For patients who do not have valid medical records from within the last 12 months, we do have doctors onsite who can perform physical exams to diagnose some medical conditions. This exam is limited to muscular or skeletal pain ailments that have been chronic (6 months or longer). 

 If, through this focused physical exam, a doctor is able to diagnose one of the Arizona Qualifying Conditions, we can assist you.

3. Set up an appointment

You must have one of the following medical conditions:

• Agitation of Alzheimer’s Disease

• Amyotrophic Lateral Sclerosis

• Cancer

• Crohn’s Disease

• Glaucoma

• Hepatitis C

• HIV/AIDS• PTSD

• Cachexia or Wasting Syndrome

• Severe Nausea

• Severe and Chronic Pain

• Seizures

• Severe and Persistent Muscle Spasms

Patients with Medical Records

Arizona Department of Health requires medical records that are dated within the last year and state a qualifying condition. (for PTSD the records have to be dated within 2-3 months, and state that you are currently undergoing conventional treatment)

You may request your records from your doctor, or download them from your healthcare portal. We do not request records for you.

4. After your evaluation

Once your evaluation is completed, an application needs to be processed with Arizona Department of Health at www.azdhs.gov/licensing/medical-marijuana/. The Fee is $150 or $75 with SNAP benefits for the two year card.

We can submit everything to the State of Arizona for an additional $15.00, or you can submit it yourself.

New Card: The card is electronic and you will receive an email from AZDHS with 1-5 days, letting you know you can log in to the MMJ portal, and download your new card. You can save the image to your phone, or print it out and keep it in your wallet.

Renewal: When you renew your Medical Card, Arizona will not update the card in your portal until the old one expires. The new one will automatically appear at that time if your application to renew has been approved. (That’s how both cards are still good for 2 years)

MJJ RENEWALS

Renewing your AZ MMJ card at our office is almost the same as getting your first card. You should receive a notice 90 days before your card expires, giving you plenty of time to get your paperwork together before the deadline. We supply you with everything you need to submit:

  • A photograph taken that day

  • A copy of your AZ driver’s license, AZ driver’s permit, AZ ID card, or your passport

  • Patient Attestation Form

  • Medical Marijuana Physician Certification Physician Information Form

Once everything is done, if you prefer, for only $15.00 our friendly staff can even submit your application for you. You will usually receive your new card in 1-5 days via email. Cards NOW makes it easy to get safe and legal access to your medications.
