$95 is the CardsNow fee to book and pay our doctor if  you have qualifying valid medical records (link explaining valid medical records ).  Without records, if you can express that you have recurring pain in the neck, shoulders, elbows, wrists, hips, knees or ankles that will allow the doctor to certify you for an Arizona medical marijuana card.  The price without records is $125.

To help you offset your costs, our cooperating dispensary, truBLISS Organics,  will provide you with a  $200 punch card. There are 10 punches. Each punch is worth $20 off any purchase of $50 or more. You can use up to 3 punches in a day.

Not tech savvy? Want us to upload your certification and create a patient portal for you at the Arizona Department of Health? The fee is just $15. 

In order to do this for a first time patient, we need you to have a phone or tablet with you that you can read emails on.

If you have an expired card or a renewal, we need 2 things to help you upload:
1)   You have a phone or tablet with you that you can read your email on. 
2)   You must remember what email you used to set up your patient portal if you already had a card.

On the AZDHS portal, you can choose “Forgot Password”, or search your email using the search word “AZDHS”.
