At the end of the certification day you will be emailed an invite to a google drive holding your digital documents that you need to upload at State of Arizona Health Department to complete the process. Monday-Friday following your booking, you will receive an online form to tell us about you and your joint pain that will be forwarded to the doctor. Please return the form completed 1 business day before your appointment. 😊
